It looks pretty ugly and atypical. I'm sure there's another forum with a psychotic smiley we can steal, and it would fit more in line with regular smileys. Civfanatics.com has a crapload of smileys, you can take a look there and see if they have an equivalent.

Why this image?
It is from Johnny Dangerously and represents a handicapped sign for those deemed psychotic. For it to be funny you would have to see the movie, however the image itself could still be meaningful if it is agreed upon as meaning "psychotic".
Why not make it smaller?
Believe me, I have tried. Unfortunately once you go below 25 pixels in height, the image becomes distorted in such a way that it is no longer similar to the original. I could further reduce it, however it would look blurry and the mouth would not appear correctly.
The educational video
It is part of the movie and is used to trick Johnny's brother into continuing with law school and postponing having sex.

If you had to make it smaller you could shrink it until it's distorted, and then just redraw over it.

Tried that, but once you get down to pixel art it requires a certain skill/talent that I do not possess.

Just because someone has the tools to do the job it doesn't mean they are capable. My brain just doesn't do well with pixel art.

So I was about to try and make this smaller when I realized it's (#1 in GP's second post) smaller than the Sonic sprite. Exactly how small would you want it to be Syd?

I modified the  emote in paint to make it look a bit like a more standard emote, if you want... It still looks pretty much like shit though:
