Basiclly, what from any game you have played really creeped you out so much that it made it hard to sleep. The music, the levels, the enemies or even the game play itself.
I would have to say the music and the enemies from the original Metroid. Back then the game had shit loads of creepy atmosphere and encountering a new enemy like the metroids for the first time was enough to lodge my heart into my throat.
And Eternal Darkness . . . well anything from that. But i have to say it didn't nearly creep me out as much as splatter house 2. Yah i said it, the 2D 16bit game scared me more then the 3D zombie mindfuckfest.

Eternal Darkness was such a mindfuck.
The best part is when you're the monk (level 7 or  , and you go through the door to fight the "boss". Pious confronts you, the game makes you think you're in for a fight of some sort, then...

I know what you mean about SplatterHouse 2. The music combined with the nasty-looking monsters made for a generally unpleasant experience - even though I mean that in the best way possible.
SPOILER:
The part in Silent Hill 3 where Claudia eats a fetus. You don't really get to see it, but you definitely hear it and just the very idea & act of someone consuming a fetus (whole, no less) made my stomach hurt.

Obvious, but it impressed me so much at the time. The original Resident Evil.
Don't play it with the lights out.
I think it pretty much defined what's now "Survival Horror"

Playing "Silent Hill" always gave me a heart attack. But I never feared the game after I turned it off.
The Licker mirror crash in Resident Evil 2 made me jump -- as it did everyone. However, that was the last effective jump scare I've ever felt in a game since. There's been good ones, but nothing had the same effect on me as that.

That big ass dude on RE 2 that comes out of that capsule and breaks through the walls freaked me out too. I don't know what he's called, but he's fuckin scary. I think he only appears after you beat the game though.

For me, it's a little-known game called Echo Night: Beyond.
The whole game is very atmospheric. You start off completely alone on a commercial shuttle that crash-landed on the moon. Everyone around you is gone, and as you progress off the shuttle and into the big station on the moon, you begin encountering ghosts of people who have died on the station. Your task is to find out what happened on the station, why everyone is dead, all the while avoiding the hostile ghosts (who can kill you) and helping the nice ones.
It's in first-person, and you have stats on your screen including your heart rate. When you encounter hostile ghosts, your heart rate shoots up, which causes the edges of your screen to begin blacking out. If you heart rate shoots too high, you eventually have a heart attack, and it's game over. You use monitoring stations to look at video camera feeds from around the area to see ahead to find where the ghosts are to help you figure out what to do.
Really, the creepy part of it is the atmosphere. The flickering lights, the clink-clink of your boots as they echo on the metal walkway, the squeaking of a broken monitor swinging fron a few wires, the sound of your own breathing... the sense of total isolation.
I may not be describing it well, but this game is freaky and fantastic. I can't play it alone, and I can't play it at night. Needless to say, I've never finished it.

I might have to agree with you on that. Even though I have never played Eternal Darkness (though I really want to), I have Splatterhouse 2 on the Virtual Console. Laugh at me all you want, but I can't play it with the lights off at night. XP

Getting the flashlight in Silent Hill 2 gave me a nice jolt; I expected something to happen but not that.
Fatal Frame nailed to whole haunted house thing it was going for, damn ghosts would pop up just when you convinced yourself that there was nothing in the room. There was also a bunch of little things added just to fuck with your head, not Eternal Darkness level stuff but simple shadows, noises and whispering.

Ah, Fatal Frame, i saw a lets play of that game not too long ago and it was scary but it would probably be more scary if i played it myself.

Hey, the same guy who did the Fatal Frame walk through did a Condemned walkthrough too. The only part that really creeped me out was the very end when Ethan's face mutated. Ethan Thomas is a bad ass!
I'm glad to see i'm not the only one who thought Splatter house 2 was a very distartbing game.

It's different when you're in the game. Instead of most horror games which lure you into a false sense of security, Condemned keeps a constant sense of fear throughout. It's a bit of a freakout to see an enemy pop out of nowhere, and just you think you'll land a a killing blow, he grabs an electrical conduit out of the wall (unscripted), which knocks out the lights, and starts chasing at you in the dark with it. I have to give it to the developers with the ending though. After watching some guy just terrorizing you for a few hours, you get to hand out one of the most satisfying ass whoopings since Death Proof.

His name is Mr. X, and yeah, he scared me too, especially when you have to go through that one hallway with the tiger statue and there's simply not enough room to get around him, meaning that you have to take a few punches from him just to get past him.
I remember this particular moment in RE3 when I was just strolling down the alleyway between the restaurant and the city hall entrance, not an enemy in sight, then I stopped in the save room, walked out of the save room and -BAM- there's freaking Nemesis standing RIGHT next to the door.

Eternal Darkness definatley creeped me out. The first Diablo scared me alot too, I couldn't play it at night because of all the demon screaming in the dungeon.

The Thief games always had moments that freaked me out. In the original, there's parts where you're exploring tombs and ruins and shits, and suddenly these horrible looking lizard things come out of nowhere groan, and shoot poison gas at you. That used to freak me out. Also, there was one statue thing in the game that said "Beware, thief!" when you took its jewel. That creeped me the hell out too.
And Thief III had the abandoned asylum. That was creepy as hell.

Good lord, that asylum level in Thief: Deadly Shadows(for the Xbox)...okay, now THAT was a creepy place. I think I got creeped out just from going through the level, no matter where I was at, mainly just because the place just had that foreboding look, like some monster would just come out and take you down. Going into the past in that level, where those black ghost-like people were, along with the fact that to beat the level, you had to make it look like you jumped out a window to your death...ugh, I was glad when that level was over.
Also, in that same game, the level where you went through that seaside mansion or whatever, the one with the creepy widow sitting in the chair...guh, freaky as hell. I pinned an arrow to her head after I finished listening to her. Of course, the level itself wasn't that creepy, but there was this one part where I listened to a record player that played back a recording of the captain saying something before he died at sea...I dunno, listening to that just sent chills up my spine.
One more: the Diablo 3 preview video, where the characters get to the end of the level, where it looks like nothing else is around, when suddenly this enormous boss just BLASTS through the wall into the area...not SCARY scary, but it made me jump.
