I decided to answer this poll by checking various dialog boxes of various software on my Mac, until I found one with an "Okay" or "OK" button, then voting for which one of those is used.
And the result was "OK".
(Woah, I actually gave a fuck about one of Vert1's polls)

Okay is Bill Waterson's. I remember hearing a story where he had to white out "ok" because he had written that by mistake or something.
That having been said, it's contextual. If it's formal (you probably shouldn't be using it anyway), it's "okay". If it's a text message or something, it's "OK".

I've had teachers not like the use of ok in writing, saying that okay was better. Plus it seems a lot of spell checkers seem to prefer it as okay too, so that's what I tend to use just to avoid seeing the stupid red line under things.

I'm gonna be honest, "OK" makes me wanna throw somebody through a plate glass window every time I see them use it. It just looks so lazy and ugly. "Okay" is so much more elegant.
