It means that it's one of the most mentioned topics. The sidebar on Twitter shows the Top Ten Trending Topics and then one "promoted" one, which is a paid advertisement.

Yeah, Ghandi owns the number one Alexz Johnson fan site (it gets more traffic than her own), and he just interviewed her a couple of weeks ago for the second time.
