Getting Porygon in Pokemon FireRed without spending a single buck.
Reaching 999999 bucks in Pokemon FireRed (and then buying a shitload of Max Potions to get back to 0).
Beating Boss Rush on Very Hard in SSBB (I suck at those games, so I'm quite proud of this).
Getting 200% in Crash Bash.
Getting all treasures in Pikmin 2 without a single Pikmin dying (after a lot of resets)
Capturing Wriggle's Last Word in Touhou 8 on my 283rd try.

Beating Gungrave OD in Kickass mode and only dying once. (I was incredibly tired when I got the insane notion to do this. Surprisngly I actually played better when I was sleep depraved compared to when I was awake)
Getting 100% on all three Shadow Hearts games. Not the biggest achievements, I know, however I felt really proud when I accomplished them.

It happens to me all the time. Sometimes I phase out, the whole screen becomes blurry, I have no idea what the fuck I'm doing, and I play like a god.
Maybe it's because you're not thinking as much, so you don't ask yourself if you should do this or that and hesitate less.

100% in all shadow hearts is pretty impressive.

Beaten Time Crisis 2, six times over the course of a month.
First time, I spent around 30 PLN in quarters. (For your reference - for 30 bucks you can buy 1 record, two considerably-sized pizzas, get thirty seconds with a hooker or buy a bottle of some pretty nice whiskey [or three bottles of medium-quality gin]).
The second time, I paid 10.50.
The third, I paid 4.50.
Fourth, 1,50 (which is one token in my local arcade).
Fifth, 3.00 (two-player this time).
Sixth, 1.50 again.
I'm thinking I should invest in my own Time Crisis cabinet and not spend any more money in the arcade...
Oh, also: My ultimate DDR-suckage broke the machine at the arcade. Seriously, it just turned itself off and the manager had to reset it.

Thank you. It's one of the few series I've played where the sidequests needed to get the best stuff are actually really fun. I especially liked winning the man-festival, one of the funniest side-quests ever.

Chrono Cross: Rainbow weapons for mostly everyone, defeating the Bend of Time.
Final Fantasy VI: Everybody had every spell (Except for Gau. I always thought he sucked)
Unlocking EVERYTHING on Crash Team Racing
Castlevania SOTN: Defeating Shaft and Dracula with the Crissaelgrim sword and no Shield Rod spell = Alucard Shield EVER.
Final Fantasy V: Mastering EVERY Job for everyone.
Final Fantasy Tactics: Beating the Deep Dungeon sub-quest and getting the Zodiac summon on Rafa. I also got Cloud, but only for bragging rights. Boy, he's total suckage.
Final Fantasy VII: Beating Emerald Weapon with the Omnislash + Counter + Mime technique, Beating Ruby Weapon andSephiroth with TWO Master Materia sets (easy like Sunday morning)
Beating EVERY SINGLE Mega Man on the NES on the same night with 100% completion.
More to come, I'm the kind of player that explores every single inch of a game searching for secrets and other goodies.

Oh, also - beating Ansem in Kingdom Hearts 2 (or was that one? I don't remember.) - I mean the shoot'em up part. I had a controller in my hands and three friends all around me. I started talking with them and before I knew it, my friend said "Um, you can stop now, he's dead."
How do you beat a Megaman game with 100% completion? Do you mean collecting every secret item or getting 9 lives or something?

Beating Super Mario World before my entire block, and beyond... I was the one that told them that you have to throw the wind-up Koopas at Bowser to defeat him... I beat that game a record, in my area, 3 weeks before anyone else... That's a boast!! This was early-1992.

Hm, let's see...
Phantasy Star II - Killed Neifirst with Nei.
SotN - 48:20 complete time with Alucard, 11:30 complete time with Richter.
FFVII - Got all the different dates (Aeris/Tifa/Yuffie/Barrett).
Fallout 3 - I reached the door to Vault 87 (not through Lamplight Caverns), only to be surprised by what happens.
I'm sure there's others but nothing springs to mind atm.

I got Rock Band 2 this week, and on my first play through of Today, by Smashing Pumpkins, I tried expert and got 100% on the solo and gold starred it.

There's not much, but let's see...
Ninja Gaiden on the Xbox - beat it on normal without dying (second playthrough)
Legend of Zelda: Ocarina of Time - finished it with 0 deaths (third playthrough)
Persona 4 - beat the optional "ultimate boss" in New Game+
Grand Prix Legends - lapped only a few seconds slower than Graham Hill's lap record at Watkins Glen (which, in this game, is something of an accomplishment)

Beat Athena for the NES. Suck...on...that. Also I highly advise no one even attempt this. It's horrible and the ending is literally the worst ending in the history of games.

Not that many honestly, but:
- ALMOST beat The Adventures of Bayou Billy (that is, getting all the way to the end boss) without using a Game Genie or any kind of cheat whatsoever. Didn't go all the way after that because I was 12 back then and I got fed up with the game at that point.
- Beat Illusion of Gaia in under 12 hours. Straight. No breaks whatsoever.
- Played Bust a Move (Bust a Groove in the US) without the aid of the indicator (there was an option in which you could turn it off).
- Beat House of the Dead 1 and 2 (arcades) with one credit each.
- Be the first person at an anime convention to complete a recently imported, just opened Katamari Damaci.
- Rent (not buy) about 10 NES games in less than a week (hey, I was young back then...)
